  1. Transfer X-rays to your new dentist.

Your new dentist will provide the necessary paperwork to transfer your X-rays from your old dentist.

  1. Fill out medical history and allergy forms.

These forms should be straight-forward, but it will take you approximately 15 minutes to complete during your first appointment with your new dentist.
