We are pleased that you have chosen us to help you. Because we want to establish a relationship of mutual understanding with you, we have compiled this letter to provide you with information about how our practice functions. PRACTICE PHILOSOPHY Our goal is to help you attain the highest level of dental health possible. With your co-operation, the benefit of a pleasing, comfortable and functional dentition can be achieved. We will always present the most ideal plan treatment for your specific situation. We believe that you should be informed of the best that modern dentistry can do for you. Remember, however, that the “ideal” does not mean “the only”. Our policy is to discuss with you all the options for dealing with your care. TEAM APPROACH Our office functions as a team. We are very proud of the training, skill, and ability of all our members. As part of your total care, each has specific duties and responsibilities. Our Receptionists are in charge of appointments, insurance and financial arrangements. Our Dental Assistants work directly with the Dentists. Our Registered Hygienists, in conjunction with the Dentists, are responsible for our Periodontal Maintenance Program. Both are involved with providing care throughout each phase of periodontal treatment. While each member of our team has individual responsibilities, we will stand ready to do whatever might be necessary to help you at all times. APPOINTMENT SCHEDULING We see patients by appointment. All treatment, to be effective, requires the scheduling of your appointments at precise time intervals. Only through this approach can you achieve the optimum response from our care. APPOINTMENT CO-OPERATION We will attempt to accommodate your circumstances in the scheduling of treatment appointments. Our appointments are such that when we reserve an appointment for you, it is not possible to schedule other patients at the same time. If you do not keep your appointment, reserved exclusively for you, that time is not used. Any change affects many patients. If your scheduled appointment must be changed, please advise our office at least TWO BUSINESS DAYS INADVANCE. In the event of an emergency, please give us the maximum notice possible. This is necessary in order for our office to function efficiently for your benefit. EMERGENCY COVERAGE At any time, if a problem arises, please contact our office. FEES Our fees are at the established current Ontario Dental Association Fee Guide. Payment is due at the time of service unless other arrangements have been made. Financial arrangements for all large cases must be made in advance of treatment. For your convenience, we accept all major credit cards, Interac and cash. DENTAL INSURANCE As many of our patients have some form of dental insurance, we will be glad to complete the treatment portion of the dental insurance form. Our professional services are rendered to you, not to the insurance company. We do not accept assignment of insurance benefits; therefore you are directly responsible for the payment of fees. If your insurance company does not completely cover the fee for a certain procedure, it is usually due to one of the following reasons: a) The procedure is not covered under your plan. b) Your plan only covers a percentage of that procedure. c) Your plan is not covering current Ontario dental Association Fees. d) Your annual maximum allowance has been exceeded. e) Some insurance plans limit the frequency of certain procedures in a given year. Although we will assist you as much as we can, it is impossible to keep up to date on every available insurance plan. Please direct your insurance inquiries to your insurance company. FINANCIAL ARRANGEMENTS We request that all of our patients take care of their account balance following each visit. Should this request present a problem, please inform the Doctor or Receptionist and alternative arrangements can be discussed. REFERRALS Many people think that in a busy practice we do not accept new patients. This is not so. We are always accepting new patients and are glad to see family and friends.
